Dose by Weight

Dose = Weight (kg) × Dose per kg

Variables: Weight in kg, prescribed dose in mg/kg

Example: 70 kg × 5 mg/kg = 350 mg

Dose by BSA

Dose = BSA (m²) × Dose per m²

Variables: BSA from nomogram or Mosteller formula

Example: 1.73 m² × 100 mg/m² = 173 mg

Mosteller BSA

BSA = √(Height(cm) × Weight(kg) / 3600)

Variables: Height in cm, Weight in kg

Example: √(170 × 70 / 3600) = 1.82 m²

Ideal Body Weight (Male)

IBW = 50 + 2.3 × (height in inches − 60)

Variables: Height in inches

Example: 50 + 2.3 × (70 − 60) = 73 kg

Ideal Body Weight (Female)

IBW = 45.5 + 2.3 × (height in inches − 60)

Variables: Height in inches

Example: 45.5 + 2.3 × (65 − 60) = 57 kg

Adjusted Body Weight

AdjBW = IBW + 0.4 × (TBW − IBW)

Variables: IBW = ideal, TBW = total body weight

Example: 73 + 0.4 × (100 − 73) = 83.8 kg

Clark's Rule (Pediatric)

Child dose = (Weight lbs / 150) × Adult dose

Variables: Weight in pounds, adult dose

Example: (50 / 150) × 500 mg = 167 mg

Young's Rule (Pediatric)

Child dose = [Age / (Age + 12)] × Adult dose

Variables: Age in years, adult dose

Example: [6 / (6 + 12)] × 500 mg = 167 mg
